shitty day.

miss A.T. 28 year old female,weighs 80 kgs. type 1 respiratory failure. intubated, mechanically ventilated & a slow wean, then got a tracheostomy. now ventilates via trachy mask with intermittent periods on the non invasive ventilator at night.

social history includes IV drug abuse, massive alcoholic beverage intake (2 liters vodka/day + cider). this made sedation extremely difficult.

--yan.. yan ang usual endorsement namen sa pasyente namen for almost 2 months. sakin nya napipiling magtoxic, magundergo ng scans at magrestless at magdesaturate ng bonggang bongga!!

one fine night (or so we thought), step down na cya sa high dependency (hdu)..pasyente ko na naman cya.PETIX NA!!konti lng ang gamot at infusions.may konteng sedation at antibiotics.may lactulose at senna at 10pm.

for some reason,natoxic kame sa HDU, so madalian portion na.. LACTULOSE 60cc per NG. ksama ng mga ibang gamot.nagbuhos ako ng lactulose ksama ng ibang 10 o'clock meds tpos binigay ko na.

'hmm, parang 60 cc na ata yun.. or not?? naku napasobra ata. haha! yaan na.. lactulose lang naman. hindi nga to tinatablan ng suppository eh' .. kahit na mukhang napasobrahan ko ang paglalagay ko, hahaha! konti lang naman. approx ba!

12mn--nagbreak ako.pagbalik ko..super duper mega restless na ang pasyente ko!!bumababa ng bed..naghahanap ng sedation. max na kami sa clonidine. nag haloperidol na rin kame chaka midazolam. it didn't touch her. then she started pooing like hell. almost every hour. cyempre ever linis kame. life was like that until 6am--when she passed out a freaking huge amount of POO!! it was like the godfather of all poo!!

then she fell asleep.

like a baby.

moral of the story: wag gawing cooking class ang pagbebedside nursing. :p
